Our goal is to foster positive relationships with clients as well as performing skilled and non-skilled services./ph3Responsibilities - Include but not limited to/h3p** Home Health Aide (HHA) Personal Care Assistant (PCA)**:/pulliEnsure quality and safe delivery of home health care services./liliImplement clients plan of care in accordance with the Agencys process and procedures policy./liliParticipate in clients case conferences according to the Agencys policies and regulations./liliSubmit updates and relevant information regarding the clients plan of care to the registered nurse in a timely manner./liliHHA/PCA will work with patients of all ages that require care in their homes./liliHHA/PCA must be able to carry out patient care plan activities./liliHHA/PCA must be caring, patient and compassionate caregiver./liliHHA/PCA must be able to accompany patients to doctors offices or to hospital visits./li/ulh3Qualifications/h3ulliHigh school diploma or equivalent preferred./liliHome health aide certification required./lili1 year home health care experience.
