Are you a Nurse Practitioner who enjoys delivering personalized, patient-centered care in community settings? This opportunity offers the ability to provide high-quality primary care services directly to patients in their homes and senior living communities while maintaining a flexible schedule.

Role Overview

This position focuses on delivering primary care services through a combination of home visits and visits to senior care environments such as assisted living and long-term care communities. You'll oversee patient care plans, complete post-discharge assessments, and work closely with scheduling teams to manage your daily patient visits efficiently.

Key Responsibilities

• Provide patient care through home visits (approximately 85%) and senior care facility visits (approximately 15%)
• Perform comprehensive evaluations, follow-up appointments, and ongoing care management
• Diagnose medical conditions and develop individualized treatment plans
• Prescribe medications and order appropriate diagnostic testing
• Coordinate referrals and collaborate with scheduling teams
• Educate patients and family members on treatment plans and preventative care strategies
• Track patient progress and adjust care plans as needed
• Complete EMR documentation within 24 hours of patient encounters
• Consult with the Medical Director when clinical guidance is needed

Schedule

• Flexible 3-day workweek
• Providers have autonomy to set their own schedules
• Typical hours are 8:00 AM - 5:00 PM
• No on-call responsibilities
• Two-week orientation program
• Credentialing may take up to 30 days
• Optional shadowing available

Service Areas

• Richmond, VA (up to a 40-mile service radius)
• Fredericksburg, VA (up to a 30-mile service radius)
• Tappahannock, VA / Northern Neck (up to a 40-mile service radius)

Required Qualifications

• Active Nurse Practitioner license in Virginia
• Current DEA license
• Active CAQH profile and CAQH number
• Minimum of 1 year of Nurse Practitioner experience in primary care, family care, senior care, assisted living, or long-term care settings
• Experience using electronic medical record systems

Preferred Qualifications

• Additional experience in urgent care, emergency medicine, trauma, or inpatient acute care settings
