Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours M-F, flexible times Experience: 1 year of experience required in long term care or rehabilitation License: Active Occupational Therapist license required Must-Have: - 1 year of Occupational Therapy experience - Long term care or rehabilitation experience - Active Occupational Therapist license Description: Effectively screens and evaluates patients with cognitive and/or activities of daily living physical functioning disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practices to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients. Administers various procedures as part of the rehabilitation plan. Includes activities such as activities of daily living retraining, therapeutic activities, therapeutic exercises, and the use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record. Evaluates treatment outcomes for effectiveness. Modifies services in relation to patient evaluation and makes referrals based on outcomes. Provides consultation and counseling to patients, families, caregivers, and other service providers. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services delivered. Documents all regulatory and clinically required information in the electronic documentation system. Maintains patient records according to company policy. Accurately records treatment minutes for services provided. Participates in required meetings such as patient care conferences and rehabilitation meetings. Engages in facility and company required trainings, in-services, and conferences. Able to obtain any necessary durable medical equipment or supplies. Supervises and supports occupational therapist assistants and rehabilitation aides or students. Adheres to professional practice acts and ethics during professional practice.
