The Youth Support Specialist provides youth-guided, strengths-based, and recovery-oriented support to children, youth, transition-age youth, and families receiving Behavioral Health Home Services. The position promotes youth voice, self-advocacy, wellness, natural supports, engagement in services, and connection to community-based resources.The Youth Support Specialist works as part of a multidisciplinary Behavioral Health Home team and collaborates with care coordinators, clinical staff, nurse care managers, psychiatric consultants, family members, caregivers, schools, medical providers, community partners, and other natural supports as appropriate and authorized.DUTIES:Engages youth and families through direct contact, outreach, education, support, and participation in team-based planning.Directly encourages and supports youth voice, self-advocacy, choice, and participation in service planning and decision-making.Builds supportive, respectful, and developmentally appropriate relationships with youth through one-on-one meetings, telephone contact, telehealth when appropriate, home/community visits, and group-based activities.Supports youth in identifying personal goals related to wellness, recovery, education, family relationships, community connection, health, independence, and transition planning.Assists youth and families in connecting to natural supports, peer supports, wellness and recovery resources, community activities, medical care, behavioral health services, educational supports, and other resources of their choice.Works collaboratively with the Behavioral Health Home team to support whole-person care, including coordination of behavioral health, physical health, social, educational, and community-based needs.QUALIFICATIONS:Per MaineCare Section92.03-1, C5, a Youth Support Specialist for Child BHH is:An individual who is between eighteen (18) and thirty (30) years old who is receiving or has received services and supports related to the diagnosis of behavioral health condition, is in recovery from that condition, and who is willing to self-identify on this basis with members receiving services in this Section. High school diploma or equivalent required; two-year degree or some college preferred. Must meet all certification, training, and continuing certification requirements applicable to the assigned Behavioral Health Home service model.
