RN Inpatient Care Coordinator

Job Summary:

The Registered Nurse (RN) Inpatient Care Coordinator provides professional nursing care across the lifespan in an inpatient setting, delivering high-quality, culturally sensitive, and patient-centered services. The RN performs comprehensive patient assessments, develops individualized care plans, administers medications and treatments, and collaborates with interdisciplinary teams to promote optimal patient outcomes, safety, and positive patient experience. This role demands strong clinical judgment, effective communication, and the ability to manage fluctuating patient acuity with competence in specialty equipment. Responsibilities:

• Perform comprehensive patient assessments and develop individualized plans of care.
• Implement and evaluate nursing interventions and patient responses.
• Administer medications and treatments safely and accurately per provider orders and policy.
• Recognize changes in patient condition and initiate appropriate interventions or escalation of care.
• Provide patient and family education related to care, medications, and discharge needs.
• Deliver culturally responsive, patient-centered care.
• Exercise independent nursing judgment in determining appropriate nursing actions and priorities.
• Collaborate with providers and interdisciplinary team members to coordinate care and facilitate referrals and follow-up.
• Document assessments, interventions, and outcomes accurately and timely in the electronic health record.
• Advocate for patient care and assist patients in navigating the healthcare system.
• Adhere to regulatory, safety, and infection prevention requirements.
• Participate in quality and performance improvement activities.
• Provide guidance and delegation to ancillary staff; support orientation, mentoring, and education of staff and students.
• Float to other departments or assignments as needed.
• Maintain required clinical competencies and certifications.

Required Skills & Certifications:

• Associate's degree in Nursing from an accredited program.
• Current, unrestricted Registered Nurse license in the State of Alaska.
• Basic Life Support (BLS) certification.
• Advanced Cardiovascular Life Support (ACLS) certification.
• Pediatric Advanced Life Support (PALS) certification.
• Clinical competency certification within 3 months of hire and every 3 years thereafter.
• High Risk Competency certification within 3 months of hire and annually thereafter.
• Strong clinical assessment, nursing judgment, and decision-making skills.
• Proficiency with electronic health records and clinical systems.
• Ability to lift, push, pull, and move up to 50 pounds and perform physical tasks required in clinical settings.

Preferred Skills & Certifications:

• Experience in acute care, chronic care, and post-operative patient management.
• Additional specialty certifications relevant to practice area (e.g., NRP, TNCC, ENPC) within six months of hire.
• Experience working in a fast-paced, multidisciplinary healthcare environment.
• Strong organizational, prioritization, and communication skills.

Special Considerations:

• On-call duties may be required.
• Must be competent in the use of all specialty equipment relevant to the practice area.
• Compliance with Alaska Board of Nursing scope of practice and organizational policies is mandatory.
• Ability to work in a moderate-to-high noise clinical environment.

Scheduling:

• Work schedule includes inpatient shifts; specific shift types and hours determined by department needs.
• Flexibility to float to other departments or assignments as needed.
