This is performed in accordance with physician or advanced practice practitioner's orders and plan of care under the direction and supervision of the Home Health leadership team.In this role you will be responsible for:Provides direct physical therapy services planned, delegated and supervised according to directions of physical therapist and in accordance with the physician's plan of care.May treat patient to relieve pain, develop or restore function, and maintain maximum performance.Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait, and functional training, transfer activities and prosthetic training.Utilizes physical agents, including but not limited to heat, cold, water, light, electricity, ultrasound and massage.Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in patient's condition.Instructs patients and families in care and use of DME.Maintains necessary records; clinical, conference, and progress notes, which will be incorporated into the patient's clinical record in prescribed time framesRequired QualificationsSuccessful completion of a physical therapy assistant program at a school of physical therapy approved by the American Physical Therapy AssociationPhysical Therapy Assistant License recognized in the state in which practicingDemonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populationsDemonstrates knowledge of the principles of growth and development over the life spanAble to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needsDemonstrates computer skills sufficient to use the electronic medical record effectivelyReliable transportation and valid automobile insuranceDesired Qualifications2 years experience working as a Physical Therapy Assistant - hospital and/or home care experience preferredLocationOffice Location: Grand RapidsMain Service Area: Grand Rapids, MIHoursFull Time8:00 am – 5:00 pm, Monday through FridayWeekend rotationPay Range$30 - $34 USDHow We Care for YouMinimum of 3 Weeks Paid Time Off (PTO)Medical, Dental, and Vision InsuranceHSA and FSA options including Dependent CareCompany paid Short Term DisabilityCompany paid Life Insurance401(k) with Employer MatchMileage ReimbursementCompany Vehicle Program for field rolesPet InsuranceID and Fraud ProtectionAnd more...Background ScreeningEmployment is contingent upon the successful completion of a background check.
