Clinical Spine Coordinator (RN), FT, Days, VUH

Job Summary Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the care coordination model. This position improves outcomes by reducing all‑cause hospital readmissions and coordinating episodes of care among patients in a defined population or disease process. Participates in identification of appropriate patients; encourages patient and family engagement in self‑care management; promotes warm handovers to the next level of care by providing timely, pertinent information in a standardized way; conducts patient and family education on key elements of the patients’ personal care plan by using the teach‑back methodology and follow up phone calls; and assists the patient in navigating the healthcare system. Spine Program Coordinator The Spine Program Coordinator plays a critical role in supporting the spine center by ensuring program excellence, regulatory compliance, and continuous quality improvement. This role partners closely with the Medical Director and interdisciplinary teams to monitor performance, drive improvements, and maintain readiness for accreditation surveys, including The Joint Commission (JCAHO). Key Responsibilities Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical outcomes. Participates in identification of appropriate patients and encourages patient and family engagement in self‑care management. Promotes warm handovers to the next level of care by providing timely, pertinent information in a standardized way. Conducts patient and family education on key elements of the patients’ personal care plan by using the teach‑back methodology and follow up phone calls. Assists patients in navigating the healthcare system. Coordinates the evaluation process of the defined patient population and may complete and document portions of the evaluation process. Supports patient access by serving as a liaison between the referring provider’s office and Vanderbilt provider. Interacts routinely and effectively with the clinical team to develop a collaborative plan for the coordination of patient care from the anchor hospitalization through the defined care episode. Develops and manages the processes related to pre‑admission and post‑discharge care transitions; establishes relationships/clinical pathways with providers/agencies to optimize care for the defined patient population. Assists in the development and dissemination of patient education materials/information, including creation of customized medication grids with input from the pharmacist/team as needed. Assists with discharge/transition planning in collaboration with the multidisciplinary team; actively engages outpatient care providers by sharing hospital course, concerns, pending test results, learning needs, partnership opportunities, etc.; and coordinates handovers including outpatient care coordinators, home health care nurses, cardiac rehab, skilled nursing facilities, etc. Demonstrates reflective practice by constantly evaluating care coordination and supports the development of protocols for practice based on evidence. Provides additional duties as assigned. Technical Capabilities Evidence‑Based Practice (Advanced): Recognizes, implements, and evaluates practice changes based on published research. RN Access Patient Education (Advanced): Demonstrates uppermost levels of patient education in complex situations and takes a lead role in achieving results. Nursing Patient Assessment & Evaluation (Advanced): Demonstrates expert knowledge and experience in patient assessment and evaluation in complex situations, and mentors less experienced peers. Trend Analysis (Intermediate): Demonstrates mastery of trend analyses and recognizes the impact of seasonality and randomness to forecast future demand. Required Experience Minimum of 5 years of clinical RN experience. Spine or neurology‑related experience preferred. Prior experience with Joint Commission (JCAHO) standards and survey readiness required. Skills & Competencies Strong project management and organizational skills. Ability to manage, analyze, and present data effectively, including dashboards and reporting tools. Demonstrated ability to lead cross‑functional collaboration and drive outcomes. Excellent communication, facilitation, and relationship‑building skills. Detail‑oriented with a focus on quality, compliance, and continuous improvement. Work Schedule Monday – Friday, 8:00 AM – 5:00 PM. Flexibility required based on program needs. No weekends or holiday requirements. Benefits Affordable high quality health plan options.
