Responsibilities
- Conduct initial evaluations and ongoing reassessments for residents requiring physical therapy services
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
- Deliver PT services, including gait training, ROM, strengthening, endurance activities, and functional rehabilitation
- Monitor progress, document outcomes in the medical record, and adjust plans as appropriate
- Educate residents and families on home exercise programs, safety, fall prevention, and activity modification
- Collaborate with nursing and therapy colleagues to optimize care and participate in care planning and interdisciplinary rounds

Requirements
- Active Physical Therapist license to practice in Pennsylvania
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing facility setting
- Willingness to travel for a 13-week assignment
- Strong documentation and interprofessional communication skills
