Physical TherapistTitle: Physical TherapistLocation: RaleighSalary Range: $68-$73 per pointDescription: We are looking for a Home care Physical Therapist. The Home Care PT evaluates the patient functional needs, creates customized treatment plans, and delivers one-on-one therapy to help individuals recover from surgery, injury, or chronic illness while living at home.ResponsibilitiesPatient Evaluations: Conduct initial assessments, start-of-care visits, and regular re-evaluations to measure functional independence and progressCustom Care Plans: Design and update personalized treatment plans under a physician's direction.Therapeutic Interventions: Provide gait and balance training, muscle strengthening, pain management, and instruction on using assistive devices (walkers,canes).Home Safety Assessments: Inspect the patient's living environment for safety hazards and recommend necessary modifications or accessibility tools.QualificationsDoctor of Physical Therapy (DPT) degree from an accredited programMinimum of 1 year of clinical or home health care experience preferredValid driver's license, reliable personal automobile, active auto insurance, and willingness to drive to patients homesEffective written and verbal communicationAbility to stand, bend, lift and transfer patients (frequently lifting up to 50-100 lbs)US Citizen or Green Card holderCurrent, active, and unrestricted Physical Therapist state license
