Details Hospital-based Case Management department Staffed with RN Case Managers and Social Work Case Managers Case load ratio of at least 25:1 per case manager Day shift schedule from 8:00 AM to 4:30 PM Required to work every other weekend and rotate holiday coverage with hospital employees Uses Meditech and Midas EMR systems Requirements Active FL or compact RN license required BLS certification required Minimum 1 year of recent experience in case management required Meditech experience required Midas experience required Complex discharge planning experience preferred No employment gaps of 90 days or more within the past 12 months Additional Information Coordinate patient care planning and complex discharge planning Navy blue scrubs required for RN staff Must reside outside a 50 mile radius of the facility, local candidates will not be considered First-time travelers will be considered for this role American Traveler Job ID P-750060.
