Minimum Requirements Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active WV State Physical Therapy License required to start the assignment. BLS Certification from AHA or ARC may be required. Home Health experience preferred. Candidates without home health experience will be considered if they complete the COS-C exam course (OASIS/COS-C) prior to beginning the assignment. Candidate must agree to a 26-week contract. Submit an up-to-date resume to be considered. Preferred: prior experience with outcome measures and OASIS documentation in home health settings.
