Hybrid RN Case Manager

Hybrid RN Case Manager role supporting aging adults through community-based care coordination and individualized care planning. Opportunity to work independently without bedside nursing demands. Role focused on continuity of care, service coordination, and long-term patient support within home-based setting. Hybrid structure combining community visits with remote care coordination responsibilities. Mission-driven senior care environment with strong interdisciplinary collaboration.

Schedule

Monday-Friday schedule. 8:00 AM-4:00 PM hours. Three community-based days weekly. Two remote workdays weekly. No nights. No weekends. No call responsibilities.

Compensation & Benefits

Base salary: $84,000-$108,000 annually based on experience. Health insurance with 75% employer-paid premium. Dental and vision insurance. 26 combined PTO days. 13 paid holidays. Mileage reimbursement. 403(b) retirement plan. 3 CME days with $3,000 reimbursement. License renewal reimbursement. Life insurance coverage. Disability coverage. Flexible Spending Account (FSA) plans. Legal assistance benefits.

Role Responsibilities

Develop individualized patient care plans. Coordinate community-based services and continuity of care. Conduct patient visits within community and home settings. Manage follow-up and ongoing care coordination remotely. Collaborate with interdisciplinary care teams. Support aging adults living independently at home.

Requirements

Active Massachusetts RN license REQUIRED. Minimum 2 years RN experience REQUIRED. Home care experience preferred. Strong communication and organizational skills REQUIRED. Comfortable working independently in community settings REQUIRED. Team-based care approach REQUIRED. Comfortable with hybrid remote/community workflow REQUIRED.
