Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across an assigned territory
• Evaluate, develop, and implement individualized patient care plans to meet therapeutic goals
• Meet a productivity standard of 30 points per week
• Document all patient care activities accurately and in a timely manner using HCHB electronic medical records
• Participate in on-call and weekend coverage as required
• Serve patients across a defined service territory including Capistrano Beach, Rancho Mission Viejo, Ladera Ranch, San Clemente, and San Juan Capistrano

Required Qualifications

• Active and unrestricted Physical Therapist (PT) license in the state of California
• Minimum of 1 year of home health experience with a Medicare-certified facility
• Proficiency with HCHB electronic medical records system
• Ability to meet weekly productivity requirements
• Availability for on-call and weekend responsibilities

Preferred Experience / Skills

• Experience managing a home health patient caseload across a multi-city territory
• Strong time management and independent clinical decision-making skills
