About the Position

Job Title: RN Case Manager Profession: RN Case Manager Specialty: RN Case Manager Duration: 8 weeks Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum years of experience required License: RN Certifications: BLS Description: Experienced case manager. Responsible for initial assessments and utilization reviews. Leads and documents interdisciplinary team meetings. Engages in patient and family interactions. Sets up skilled home care services and necessary vendors for oxygen and respiratory equipment. Handles enteral nutrition and total parenteral nutrition (TPN) requirements. Arranges for hospital beds as needed. Participates in care plan meetings.
