Regional Nurse Practitioner: In-Home Care with Travel
Optum
Optum Care at Home (OAH) invites a Nurse Practitioner to join the team delivering longitudinal, hands-on care to medically complex patients in their homes. You will work within an interdisciplinary team to prevent avoidable hospitalizations and improve quality of life.
