We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team in Haskell, NJ.

About the Role:

As a key member of our clinical team, you will provide on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation. Our goal is to minimize hospitalizations, elevate patient care standards, and increase reimbursement for your facility.

Responsibilities:

• Provide comprehensive geriatric or internal medicine care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, with support from a dedicated scribe team

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with other healthcare professionals to ensure seamless care transitions and coordination

• Develop and implement individualized care plans for patients

• Monitor patient progress and adjust treatment plans as needed

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Masters degree in Nursing or related field
