White Glove Community Care is actively seeking a Patient Coordinator to join our growing team. If you have experience in home care coordination, healthcare scheduling, case management support, or patient services , this is your opportunity to grow with a mission-driven organization.

Key Responsibilities
· Coordinate and manage home care cases and caregiver scheduling
· Serve as the primary point of contact for patients, families, and caregivers
· Handle high-volume calls , follow-ups, and patient inquiries
· Ensure accurate documentation and compliance in HHA Exchange
· Collaborate with nurses, case managers, and clinical teams to support Patients needs
· Monitor attendance, coverage, and missed visits
· Maintain daily, weekly, and real-time updates on case status
· Provide excellent customer service in a fast-paced healthcare environment
Qualifications
· 2+ year experience in home care, healthcare coordination, patient services, or medical office setting preferred
· Strong knowledge of home care operations, scheduling, or case coordination
· Experience with HHA Exchange, EVV systems, or healthcare EMR platforms is a plus
· Excellent communication, multitasking, and problem-solving skills
· Ability to manage high-volume workload and prioritize effectively
· Bilingual (Spanish/English) preferred
