Description: The primary functions of an LPN include assisting with assessment, planning, implementing, and evaluating the care of all patients, regardless of age, during the shift. Responsible for meeting the established standards of skilled nursing practice. The Licensed Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients. Effectively provides person-centered care and services. Administers medications and treatments under the supervision of a Registered Professional Nurse in accordance with facility policy. Takes the patient's vital signs such as respiration, temperature, blood pressure, and heart rate, and reports changes in the patient's/resident's conditions. Practices ethically and professionally within their scope and advocates for patients. Maintains clinical competence specific to their care area, including organization-required education within the stated timeframe. Coordinates care with the interdisciplinary team. Documents appropriately and thoroughly in EMR/Point Click Care. Keeps nursing personnel and the responsible RN informed regarding changes in the condition of patients/residents and other pertinent observations. Every other weekend is a must, as well as every other holiday. Floating to other units may be required as staffing permits. May be needed to drop to CNA position as staffing permits and needs. This position is a 40-hour, 5 evening per week position. Staffing ratios for this shift are 2 LPNs to 3-4 CNAs for 40 residents. Typical orientation period will be 1 day on the assigned unit. Scrubs of any color may be worn by the employee.
