Nurse Practitioner

Title: Nurse Practitioner - Primary Care | Tucson / Avondale, AZ


Summary:
A rapidly growing healthcare organization focused on value-based primary care for the senior population is seeking an experienced Family Nurse Practitioner to join its outpatient team in Tucson/Avondale, AZ. This role offers longer patient visits, smaller patient panels, and the opportunity to build meaningful relationships while delivering high-quality preventative care in a collaborative clinical environment.


Day to Day:

• Provide outpatient primary care services to adult and geriatric patients
• Conduct comprehensive assessments and develop treatment plans
• Prescribe medications and manage chronic and non-complex acute conditions
• Work closely with physicians and interdisciplinary care teams
• Focus on preventative care, chronic disease management, and reducing unnecessary ER visits
• Utilize eClinicalWorks EMR
• Daily schedule includes:
  • 8:00am team huddle
  • 12:00pm-1:00pm lunch
  • 3:30pm last patient

Qualifications:

• Active Arizona NP license (or in process of obtaining)
• Family Nurse Practitioner (FNP) certification required
• BLS or ACLS certification required
• 2+ years of NP experience preferred
• Primary care experience required
• Geriatric experience strongly preferred
• Value-Based Care and/or Managed Care experience preferred
• Background in Family Medicine, Internal Medicine, or Geriatrics preferred
