Key Responsibilities

• Conduct thorough physical therapy evaluations for children, assessing their physical capabilities and challenges.
• Develop and implement individualized treatment plans designed to promote optimal recovery and rehabilitation for each client.
• Continuously reassess clients' progress, adapting treatment plans based on clinical signs and patient feedback to ensure their ongoing success.
• Maintain accurate documentation through our electronic record system, including SOAP notes and progress summaries, all completed within required timelines.
• Educate patients and their families on the recovery process, addressing potential challenges and establishing realistic expectations for progress.
• Meet or exceed minimum utilization goals and other business objectives as outlined by the clinic management.
• Collaborate effectively with colleagues across different therapy disciplines within the clinic for clients receiving multiple services, ensuring a seamless continuum of care.
• Embody the clinic's core values and advocate for our organizational culture, acting as a representative for Musculoskeletal Care Rehab Clinic.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, ranging from children to adults, encompassing a variety of diagnoses and backgrounds. Your expertise will be crucial in tailoring therapy approaches that account for the individual needs of each patient, fostering an inclusive and supportive environment that promotes recovery and independence. Qualifications

• A collegiate education culminating in a Master's or Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Current, unrestricted licensure from the Board of Physical Therapy in New Jersey.
• A commitment to providing therapy in both in-person and telehealth formats, when appropriate, ensuring access to care in any setting.
• A strong desire to work with a diverse range of patients, understanding and respecting their unique backgrounds.
• Flexibility and adaptability to navigate the evolving landscape of healthcare, particularly in a home care context.
• A team-oriented attitude with a focus on customer service and collaboration across all interactions.

Preferred Experience

• Previous experience in home care settings, particularly working with pediatric populations, is highly valued.
• Knowledge of and proficiency in using electronic health record (EHR) systems to track patient progress and documentation.
• Experience in multidisciplinary teams is beneficial to foster effective communication and collaborative care.
