Job Summary

**FIRST REVIEW OF APPLICATIONS MAY 14TH
The CAC/CADI/TBI Social Worker coordinates the assessment of clients' functional skills and needs and assesses the supports and services available to meet client needs and preferences. Develops, revises, and implements individual service plans and ensures coordinated approaches to services among all providers. Conducts MnChoices Assessments and Reassessments.

*Rice County is currently hiring for 2 CAC/CADI/TBI positions.

Examples of Duties

• Provides case management services to clients in Community Alternative Care (CAC), Community Alternatives for Disabled Individuals (CADI) and Traumatic Brain Injury (TBI) Home and Community Based Services programs.
• Manages waiver and service agreements, authorizes purchases of services, monitors expenditures, participates in contract negotiations, and directs representative payee accounts.
• Attends training sessions and staff/community meetings.
• Arranges transportation and travels to client meetings; attends medical and psychiatric appointments with clients as necessary, within and outside of County as called for in the individual plan of care.
• Provides back-up intake support for the County; follows up on child protection and/or vulnerable adult reports, investigations, etc. to ensure that client risks are minimized.
• Performs other duties as assigned.

Qualifications

MINIMUM QUALIFICATIONS:
This position requires a Bachelor's degree in social work or a related human services field, and one year of supervised experience working with people with developmental, mental health or physical disabilities. A valid Driver's license required upon hire. MnChoices certified Assessor through the State of Minnesota. Valid Driver's License required.


PREFERRED QUALIFICATIONS:
Master's degree in Social Work, and three years of experience as a county Social Worker is preferred. Bi-lingual in Somali or Spanish.

Supplemental Information

HIRING RANGE / UNION: $36.01 - $37.65 Per hour / SS Unit - Grade 12

HOURS: Full-time hours equal 40 hours per week. Business hours are 8:00 am - 4:30 pm Monday - Friday.
