Transitional Case Manager - Mental Health & Housing Support
Beacon Mental Health
Beacon Mental Health is seeking a Transitional Case Manager to assess needs, coordinate services, and support clients with severe mental illness. The role includes crisis intervention, housing assistance, and collaboration with treatment teams to ensure appropriate services are in place. Ideal candidates have a related degree, CPR/First Aid certification, and at least 1–3 years of relevant experience.
