Description The RN Case Manager position supports the physician and interdisciplinary team in facilitating patient care, with the goal of enhancing clinical outcomes and patient satisfaction while managing the cost of care. The role integrates and coordinates care facilitation and discharge planning functions, working in partnership with the UR RN and Social Work Case Manager. The RN Case Manager is accountable for a designated patient caseload and plans effectively to meet patient needs, manage the length of stay (LOS), and promote efficient utilization of resources. Key Responsibilities Facilitate collaborative management of patient care across the continuum. Intervene as necessary to remove barriers to timely and efficient care delivery and reimbursement. Apply process improvement methodologies to evaluate outcomes of care. Coordinate communication with the healthcare team, patients, families, and post-acute care services. Perform readmission screening and prevention. Salary range: $72,779.20 – $94,612.96 per year, depending on experience. Qualifications Associate degree (or higher). Registered Nurse license. 3 years of hospital clinical experience; minimum 1–3 years of hospital or payer Case Management experience preferred. Excellent interpersonal communication and negotiation skills. Strong analytical, data management, and computer skills. Current working knowledge of discharge planning, utilization management, case management, and performance improvement preferred. Understanding of pre-acute and post-acute venues of care and post-community resources preferred. Strong organizational and time‑management skills. Ability to work independently and exercise sound judgment in interactions with the health‑care team and patients/families. Minimum Required Experience 1 year of Case Management experience.
