Job Description

The OB RN Case Manager coordinates comprehensive care for maternal and child health populations, ensuring safe, evidence-based interventions across the continuum of care. This role focuses on intake assessments, individualized care planning, discharge coordination, and ongoing monitoring of clinical outcomes for Medicaid members. The OB RN Case Manager manages a high-volume caseload while maintaining quality standards, supports compliance with Medicaid contracts, and promotes improved maternal and neonatal outcomes within a mission-driven, collaborative environment.

Responsibilities

• Coordinate care for maternal and child health populations, with a focus on obstetrics, high-risk antepartum, and neonatal care.
• Conduct intake assessments to identify medical, social, and behavioral health needs for assigned members.
• Develop individualized, evidence-based care plans that address maternal and neonatal health needs across the continuum of care.
• Coordinate and facilitate safe discharge planning to ensure appropriate post-acute and community-based services are in place.
• Monitor clinical outcomes and adjust care plans as needed to support improved maternal and neonatal outcomes.
• Provide patient education and disease management support to promote self-management and adherence to treatment plans.
• Engage in outbound and inbound patient calls to perform follow-up, care coordination, and ongoing case management.
• Support utilization management processes by reviewing clinical information and applying coverage and medical necessity criteria.
• Ensure compliance with Medicaid contract requirements and applicable regulations for managed care and health plan settings.
• Document all assessments, interventions, and outcomes accurately and timely in electronic health records (EHR) and related systems.
• Manage a high-volume caseload while maintaining quality standards, productivity metrics, and timely completion of tasks.
• Collaborate with multidisciplinary team members, including providers, social services, and community resources, to optimize member care.
• Coordinate care transitions between inpatient, outpatient, and community settings to reduce readmissions and gaps in care.
• Utilize EHR systems and Microsoft Office tools to track, report, and communicate member status and care activities.
• Participate in training sessions, orientation, and ongoing professional development to maintain and enhance case management skills.
• Apply knowledge of Medicaid and managed care processes to ensure members receive appropriate, cost-effective services.
• Engage members using motivational interviewing techniques, when applicable, to encourage behavior change and adherence.
• Incorporate awareness of maternal health quality measures, such as HEDIS, into care planning and outcome monitoring.
• Leverage community resources such as Healthy Start, WIC, and SNAP to address social determinants of health and support members.
• Demonstrate cultural competence and patient engagement skills to build trust and improve member satisfaction and outcomes.

Essential Skills

• Active Registered Nurse (RN) license in the state of Florida.
• 35 years of clinical experience in obstetrics (OB), high-risk antepartum, or neonatal intensive care (NICU).
• 35 years of experience with case management in a managed care organization, health plan, or insurance carrier.
• Experience with utilization management processes and criteria-based review.
• Experience supporting Medicaid members in a clinical or managed care environment.
• Strong organizational skills with the ability to manage a high-volume caseload while maintaining quality standards.
• Strong documentation skills with proficiency in accurate, timely clinical and administrative recordkeeping.
• Proficiency in care coordination and discharge planning across the continuum of care.
• Ability to work independently, prioritize tasks effectively, and meet established productivity metrics.
• Experience providing patient care, patient education, and disease management support.
• Experience conducting outbound and handling inbound patient calls in a healthcare or case management setting.
• Proficiency with electronic health record (EHR) systems.

Work Environment

This position operates in a hybrid work environment based in Florida. The schedule runs Monday through Friday from 8:30 AM to 5:00 PM with a 30-minute lunch break, and offers flexibility in start times. The role requires onsite presence on Tuesdays, with additional onsite requirements as needed. Orientation takes place onsite on Mondays for computer pickup, followed by onsite training sessions on Tuesdays and Thursdays, and dedicated onsite EPIC training on Wednesdays. After the initial training period, the role transitions to a hybrid model with occasional onsite visits in Sunrise, Florida. The team works within a mission-driven culture that prioritizes improving maternal and child health outcomes, supports professional growth, and encourages collaboration. The environment relies heavily on electronic health records, EPIC, Microsoft Office, and telephonic outreach to members, with a focus on structured workflows, measurable outcomes, and supportive supervision.

This is a Contract to Hire position based out of Oakland Park, FL.

The pay range for this position is $38.00 - $44.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
