Social Worker III

Job Description - Social Worker III (2601495) At Stony Brook Medicine, our Social Workers are valuable members of our team who provide empathic care to our patients, their families, and visitors with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families, and groups in a variety of settings. Responsibilities Psychosocial assessment of Social Work High Risk Criteria Cases. Coordinate with Medical Teams, Community Partners, and Transitional Care teams. Charting and documentation, including training, data management, and statistics. Assist patient financial services, including Medicaid and other entitlement programs. Serve as liaison between community agencies and the hospital. Prepare material as needed for guardianship, oversight agencies, and surrogate decision-making. Prepare weekly updates for the Long Stay Committee. Lead therapeutic groups when appropriate. Supervise professional social work students when appropriate. Actively participate in performance improvement activities, including program development, data collection, and analysis. All other duties as assigned. Qualifications Required MSW from an accredited Social Work Education School. NYS License within one year of hire. Experience in Medical Health Care. Excellent written and oral communication skills. Ability to work and coordinate with an established Medical Treatment Team. Experience in discharge planning and Long Length of stay barriers and systems. Experience in Assessment and psycho‑social skills. Working knowledge of Long Island and Metropolitan NY community resources. Ability to demonstrate patient‑first advocacy and service excellence. Computer literate. Ability to implement and run medical team rounds. Understanding of community systems, project management, action planning, teamwork, and data collection and statistics. Demonstrated ability to manage large and complex case loads. Understanding of the role of Social Worker and Case Management as it pertains to throughput and Length of Stay. Preferred Experience in multiple medical specialties in Social Work Health Care. Experience in Hospital, In‑Patient setting, or Skilled Nursing Facility. Discharge Planning experience. Diagnosis‑specific Social Work experience. Bi‑lingual. Pre‑employment Requirements Successfully complete a pre‑employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services. Complete electronic reference checks with a minimum of three professional references. Successfully complete a 4‑panel drug screen. Meet regulatory requirements for pre‑employment screenings. Provide a copy of any required New York State license(s) or certificate(s). The salary range is an estimate at the time of posting and the specific offer will be based on the candidate’s validated years of comparable experience. Additional annual supplemental pay may include location pay, inconvenience pay, and other benefits such as generous leave, health plans, and a state pension.
