In this role, you will evaluate and treat residents to help them achieve their highest level of independence and functional mobility while delivering exceptional patient-centered care.If you are passionate about improving the quality of life for seniors and enjoy working in a collaborative post-acute care environment, we encourage you to apply.Position SummaryThe Staff Physical Therapist evaluates and treats residents, collaborates with physicians, families, and the interdisciplinary healthcare team, and maintains accurate documentation in the medical record. The Physical Therapist also provides ongoing education and in-service training to facility staff to promote quality resident care and best practices.Essential Duties & ResponsibilitiesEvaluate residents and develop individualized treatment plans based on physician orders and clinical findings.Provide skilled physical therapy services to improve strength, mobility, balance, endurance, and overall functional independence.Monitor resident progress and modify treatment plans as appropriate.Communicate effectively with residents, families, physicians, nursing staff, and the interdisciplinary care team.Maintain timely, accurate, and compliant documentation in the medical record.Participate in care planning meetings and interdisciplinary team discussions.Educate residents and caregivers on therapeutic exercises, safety techniques, and discharge recommendations.Conduct in-service education and ongoing training for facility staff.Ensure compliance with all federal, state, facility, and professional practice standards.QualificationsCurrent Physical Therapist license in the State of California.Graduate of an accredited Physical Therapy program.CPR certification preferred.Experience in skilled nursing, post-acute care, or rehabilitation is preferred.Strong communication, organizational, and clinical decision-making skills.Ability to work independently while collaborating effectively with an interdisciplinary team.Why Join Hayward Gardens Post Acute?Competitive PRN pay of $65.00–$70.00 per hourSupportive and collaborative rehabilitation teamModern therapy environment focused on quality outcomesOpportunity to make a meaningful impact in the lives of our residentsHayward Gardens Post Acute is an equal opportunity employer.
