Job Summary and Qualifications This position acts as a liaison between internal and external referral sources and acute inpatient rehab operations. The Clinical Rehabilitation Specialist is responsible for establishing relationships with case managers, physicians, payers, and other referral sources. The Clinical Rehabilitation Specialist will ensure appropriate patient placement through clinical screenings and assessments in collaboration with both hospital Case Management and the Rehab Medical Director. Job Duties and Responsibilities Assist and coordinate intake and pre‑admission screening process. Accurately document evidence of rehab admission criteria on pre‑screen evaluation in a timely and legible manner. Pre‑screen updates are completed within 48 hours of admission. Admission decisions are communicated to referral source within 2 hours. Provide information related to inpatient rehab and program features to patients and family members. Communicate to patients expectations and requirements for participation upon admission to the rehab program. Offer each patient and family a tour of the rehab unit; coordinate tour if unable to provide personally. Secure information relating to patients resources and benefits. Demonstrate clinical knowledge of rehab admission criteria, leading to appropriate identification of qualified rehab patients. Advise referral sources on inpatient rehab criteria. Function as primary contact for external and internal case management and referring physician. Establish and maintain effective working relationships with all referral sources. Complete face‑to‑face, in‑service educational contacts with physicians, case managers, other interdisciplinary members and other professionals. Serve as educational resource to patients, family members and caregivers. Participate in market analysis, data analysis and development and implementation of marketing and business plans for the Rehab Unit. Contribute to the development and maintenance of referral source information such as brochures, flyers, referrals and information kit and other outreach and marketing tools. Be proficient in time management. Prioritize meeting commitments to referral sources. Develop a call‑plan to prioritize utilization of time. Assist in developing the annual strategic plan for the Rehab Unit/Hospital, to include visible community outreach program and raise community awareness of services available. Ensure referral sources receive appropriate and timely follow‑up communications regarding their patients and hospital programs. Develop a backup system to handle calls when unavailable. Develop a system that ensures calls are appropriately handled 24 hours a day, 7 days a week. Have reliable transportation to visit referral locations. Assist as needed with PAI transmissions. Perform on‑site and off‑site clinical evaluation to assist Rehab Medical Director in determining appropriateness of patient for admission. Other duties as assigned. Skills, Knowledge and Abilities Professional appearance and compliance with dress code policy. Demonstrates excellent interpersonal skills within the facility and public. Demonstrates ability to be flexible, organized and functions well under stressful situations. Participates in weekend rotation schedule to ensure continuous access by referral sources to rehab services for evaluation and admission to program 7 days a week. Computer and keyboarding skills (25 wpm minimum). Organization and time‑management skills. Strong problem‑solving skills. Interpersonal skills. Continuous focus on excellence in work environment. Complies with all regulatory requirements including all State, Federal and Joint Commission regulations. Ensures confidentiality. Builds and maintains strong relationships with physicians, departments and staff within the facility and community. Communicates effectively and clearly. Represents the facility in a positive and professional manner. Works with others to ensure quality patient care. Supports the mission and vision of all HCA facilities in the market. Understands and is committed to helping HCA facilities achieve its goals. Learn safety policies and rules, use appropriate safety equipment and procedures. Report unsafe conditions. Check with supervisors regarding safe procedure use. Adheres to the HCA Code of Conduct and demonstrates ethical behavior as expected by our patients, colleagues and the community. Reports activity violating laws, rules or the Code of Conduct. Qualifications Position Requirements Minimum requirements:

  1. Licensed Professional in a clinical health care specialty (e.g., RN, Physical Therapist, Occupational Therapist, or Speech Therapist). Current FL RN license required, or appropriate compact licensure if position held by RN. If compact license held, active FL RN license required within 60 days of hire if position held by RN.
  2. Current state licensure in clinical practice area or eligible for state license transfer.
  3. Graduate of an accredited school in clinical specialty.
  4. Minimum 2 years recent clinical care experience.
  5. Professional appearance.
  6. Effective verbal, written and persuasive communication skills.
  7. Computer and keyboarding skills (25 wpm minimum).
  8. Ability to work with others to ensure quality patient care.
  9. Organization and time management skills.
  10. Strong problem solving skills.
  11. Interpersonal skills.
  12. Continuous focus on excellence in work environment.
Preferred requirements:
  1. BCLS certification (AHA) required within 30 days.
  2. If RN, CRRN preferred and/or achieved within 2 years of accepting position.
  3. Clinical experience in the care and management of the Inpatient Rehabilitation patient.
  4. Experience as a clinical liaison coordinating admissions for inpatient rehabilitation preferred.
  5. Persistent and persuasive when faced with challenging circumstances.
  6. Ability to analyze data related to referral opportunities.
  7. Skilled strategy planning and goal setting.
  8. Ability to build strong relationships with referral sources while being engaging, accountable and responsive.
