RN Case ManagerPosition Summary: Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.Minimum Required Qualifications:FL/compact RN licenseBLS2 years case manager experience in an acute hospital settingAcute hospital experience in specialty during the past year with no more than a 90-day breakKnowledgeable in Meditech, Midas, and Interqual.Length of Assignment: 13 weeksShift / Hours Per Week: 8:00AM-4:30PM with every other weekendSystems: Meditech, Midas, InterqualStart Date: ASAP
