Case Manager

The Case Manager provides clients with essential behavioral health case management services, focusing on implementing individualized service plans to address behavioral health needs, substance use challenges, and housing stability. This position works collaboratively with other members of the client's care team to identify and access services necessary for the client to stabilize and achieve their goals.

CSS is a Catholic human and social services agency and behavioral health care provider that welcomes people of all faiths through its programs and employment. We provide help for our Alaskan neighbors through a variety of supportive service programs, including emergency shelter, food stability, housing, case management, and refugee assistance. CSS empowers individuals and families on their path to permanent stability, creating thriving communities for all. Located in Anchorage, Alaska, CSS is a nationally accredited organization through the Council on Accreditation (COA), which recognizes that CSS provides high quality services that meet best practice standards in the social service field.

This program provides supportive services to adults who are unhoused in the Anchorage community. The program is dedicated to helping individuals move from crisis to stability, improving their quality of life, and empowering them to achieve their full potential. Through this collaborative, trauma-informed approach, we support each client in their unique journey toward healing, stability, and lasting well-being. We do this by providing behavioral health support, health and wellness, housing assistance, and peer support.

Required competencies include accountability, adapting to change, confidentiality, effective communication, cultural competence, decision making, delivering high-quality work, results orientation, teamwork, stress tolerance, empathy, and knowledge of case management models, principles, motivational interviewing, trauma informed care, and community resources.

Responsibilities include behavioral health support, resource coordination, documentation and billing, crisis intervention and support, ongoing support and advocacy, collaboration and coordination, and community resource knowledge.

Qualifications vary by level: Case Manager I requires a high school diploma or GED and one year of relevant experience; Case Manager II requires one year of experience as a Case Manager I; Case Manager III requires a bachelor's degree and four years of experience; Case Manager IV requires two years of experience as a Case Manager III. A valid driver's license, auto insurance, and personal vehicle are required for all levels.

Work environment includes a typical shared indoor office space, outreach in shelters and community agencies, and driving. Physical demands include occasional lifting, frequent sitting or standing, and travel within Anchorage. Travel out of Alaska is rare.
