Senior Wellness Center PA - Geriatrics & Home Care

P3 Health Partners in Oregon is seeking a Physician Assistant to join our Senior Wellness Center. In this full-time role, you will work with an interdisciplinary team to improve outcomes for complex geriatric patients through comprehensive assessments and coordinated primary care. You will conduct in-office visits, telemedicine as needed, and on-site assessments at senior facilities, with a focus on risk assessment, care planning, and prevention. #J-18808-Ljbffr P3 Health Partners
