Outpatient Physical Therapist

Job Description

This is a 6-month contract position starting August 17th. The Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with the patients' needs, goals, and physician direction. Responsibilities include overseeing all discipline-specific therapy services and ensuring the design of functional treatment programs for effective community reintegration. The PT will also serve as an educator for patients, families, and coworkers, while upholding professional standards and compliance with the organizational Code of Conduct.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals focused on patient discharge.
• Complete accurate and timely medical record documentation, including goal writing and quality assessments.
• Ensure services are provided under valid physician orders and within insurance approvals.
• Participate in student programs, providing a positive learning environment.
• Develop an individualized professional growth plan through continuing education and professional activities.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience.
• CPR Certification.
• Active and applicable state licensure is required.
