In this full-time role, you will be responsible for providing high-quality, comprehensive healthcare services to elderly residents within an assisted living facility setting. Work Environment

The FNP will operate onsite at an assisted living facility, collaborating closely with a multidisciplinary team of healthcare providers. You’ll enjoy a dynamic work environment that emphasizes teamwork, communication, and ongoing professional development. Our typical work schedule will be Monday through Friday from 8:00 a.m. to 5:00 p.m., with occasional evening clinics and participation in weekend health events.

Key Responsibilities

In your role as a Family Nurse Practitioner, your responsibilities will include:

• Conducting holistic health assessments and screenings for elderly patients and their families, focusing on both preventive care and chronic disease management.
• Diagnosing and managing acute and chronic medical conditions, tailoring treatment plans to meet the unique needs of each patient.
• Collaborating with community organizations to implement health education programs and improve community health outcomes.
• Providing culturally competent healthcare, advocating for patients’ rights, and facilitating access to essential health services.
• Coordinating referrals and ensuring follow-up care, maintaining continuity in the patients’ treatment journey.
• Documenting comprehensive, accurate medical records in compliance with applicable healthcare regulations and standards.
• Participating in outreach activities and health fairs to provide screenings, education, and resources to the community.
• Staying informed about current health trends, evidence-based practices, and advancements in nursing.

Patient Population / Client Focus

The primary population served includes elderly residents of the assisted living facility, many of whom may face unique health challenges such as mobility issues, cognitive decline, and chronic conditions. Your role will be crucial in not only treating their medical conditions but also enhancing their quality of life through empathetic, respectful care.

Qualifications

• Current certification as a Nurse Practitioner with an active nursing license in the state of Texas.
• A Master’s degree in Nursing from an accredited institution.
• Minimum of 3-5 years of clinical experience, ideally in community health or public health settings.
• Fluency in Spanish is essential, enabling effective communication with our diverse patient population.
• Strong interpersonal skills and the ability to work collaboratively with multidisciplinary teams and diverse patient groups.

Preferred Experience

• Experience in geriatric care or within assisted living facilities.
• Demonstrated expertise in managing chronic diseases prevalent among elderly populations.

Compensation and Benefits

We offer a competitive salary ranging from USD 110,000 to 130,000 annually, commensurate with your experience and qualifications.
