Work Environment

This role is primarily onsite at a local school facility, providing a focused environment for healthcare delivery within a community setting. The Nurse Practitioner will work alongside a diverse team of healthcare professionals and educators, establishing a supportive atmosphere that promotes patient engagement and empowers families to take charge of their health. Regular working hours are from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m., and participation in community outreach events, including health fairs, may be required during weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients of all ages, encompassing both acute and chronic conditions.
• Diagnose health issues and develop comprehensive treatment plans, ensuring the integration of preventive care strategies and health promotion activities.
• Collaborate with community organizations to devise and implement health education programs, focused on disease prevention and health literacy.
• Advocate for patients' rights and access to healthcare services, providing culturally sensitive care that respects the diverse backgrounds of the community.
• Coordinate referrals, ensuring seamless follow-up and continuity of care for patients.
• Maintain meticulous medical records that comply with legal and regulatory standards while utilizing electronic health record systems effectively.

Qualifications

• Candidates must possess current certification as a Nurse Practitioner and an active state nursing license in Texas.

Compensation and Benefits
