The Belares’ Family Care Center in San Antonio, Texas, is seeking a dedicated and experienced Family Nurse Practitioner (FNP) to join our thriving team. Work Environment

This role is conducted primarily onsite in a vibrant school environment, allowing for direct interaction with students and their families. The work schedule is from 8:00 a.m. to 5:00 p.m., with occasional evening clinics available until 8:00 p.m. Weekend events may also be part of the commitment, ensuring that our outreach efforts resonate with the community. Your average workweek will consist of 40 hours, with a supportive and collaborative team atmosphere focused on achieving optimal patient outcomes.

Key Responsibilities

As a Family Nurse Practitioner at Belares’ Family Care Center, your daily responsibilities will include:

• Conducting thorough health assessments and screenings for individuals and families, addressing both physical and mental health concerns.
• Diagnosing and managing both acute and chronic medical conditions, integrating preventive care and health promotion into your practice.
• Staying abreast of community health trends, evidence-based practices, and engaging in continuous internal training to enhance your clinical skills.
• Collaborating with local schools, organizations, and healthcare teams to design and implement effective health education programs.
• Providing culturally sensitive care, advocating for patient rights, and ensuring access to necessary healthcare services.
• Coordinating referrals and follow-up care, maintaining a seamless continuum of care for our patients.
• Participating in community outreach events and health fairs to increase awareness and offer essential health services.
• Maintaining accurate and confidential medical records in line with regulatory standards and organizational policies.

Patient Population / Client Focus

Qualifications
