This is a full‑time role with virtual work flexibility, allowing for remote coordination while maintaining in‑person support when necessary. Treatment Plan Follow‑Up : Monitor client progress, evaluate effectiveness, and collaborate with providers. Resource Navigation : Assist clients in obtaining community services, financial aid, and additional healthcare resources. Continuity of Care : Support transitions between different care settings, ensuring consistent follow‑ups and coordination. Advocacy & Education : Empower clients and families with information about mental health resources, helping them engage in their own care.
