Location: Portland, Oregon (OR)

Duration: 13 Weeks
Position Overview

We are seeking an experienced Occupational Therapist (OT) to provide skilled home health services to patients throughout the West Portland area and surrounding communities. The ideal candidate will be comfortable working independently in the home setting while delivering high-quality rehabilitative care.

Day Shift
8-Hour Days
Monday-Friday Schedule

Required Licensure & Certifications

Active Oregon Occupational Therapy License
BLS Certification

Experience Requirements

Minimum 1 year of Skilled Home Health experience required
First-time travelers accepted
OASIS experience required

Patient Caseload

One-on-one patient care
Up to 6 patient visits per day
Productivity expectation of approximately 30 points

Responsibilities

Perform occupational therapy evaluations and reassessments
Develop and implement individualized treatment plans
Provide therapeutic interventions to improve patient independence and safety
Complete OASIS assessments and required documentation
Educate patients and caregivers on treatment goals and home safety strategies
Coordinate care with physicians, nurses, therapists, and other healthcare professionals
Maintain accurate and timely clinical documentation
