Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans in accordance with the patient's needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities. The therapist will also serve as an educator for patients, families, students, coworkers, and themselves while adhering to the organization's Mission Statement and Core Values.

Responsibilities

• Complete a comprehensive evaluation of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Document patient care in a clear, concise, accurate, and timely manner, adhering to state, federal, and insurance guidelines.
• Participate in student programs as assigned, providing mentorship and evaluations as needed.
• Create and pursue an individualized professional development plan, engaging in continuing education and self-study.
• Adhere to the State Practice Act and maintain professional licensure in good standing, completing all state-mandated continuing education requirements.
• Demonstrate an understanding of performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR Certification.

Schedule

This is a 5x8.0 shift schedule with guaranteed hours.
