Physical Therapist

Outpatient Orthopedic Physical Therapist Family Health Centers of San Diego (FHCSD) is recruiting an Outpatient Orthopedic Physical Therapist to provide care for adolescent to adult patients aged 11 years and older. The position is available as part‑time or full‑time. Responsibilities Communicate with patients and/or parents/caregivers regarding evaluation results, treatment plan and goals. Provide home program activities to ensure carryover to the home environment. Engage patients and/or parents/caregivers in treatment sessions. Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards; in addition, documentation complies with patients' insurance requirements. Consult with physicians, allied health professionals, and other team members. Participate in case conferencing meetings and in program planning meetings. Facilitate training and presentations for staff, patients, and parents/caregivers when applicable. Evaluate patients referred for physical therapy and physical therapy consultation. Perform evaluations according to each patient's needs, physician recommendations, and specific program guidelines/protocols. Communicate regularly with referral, case management, care coordination, and scheduling staff. Make recommendations and follow through with referrals when appropriate. Provide appropriate oversight and supervision of Physical Therapy Assistants as necessary. Ensure that services provided by the Physical Therapy Assistants are safe, effective, and efficient. Determine when the Physical Therapist must personally render physical therapy interventions. Provide treatment to assigned patients as appropriate in accordance with established standards of care and practice. Design an appropriate and effective treatment plan and work within scope of practice at all times. Monitor progress of treatment goals through periodic observations and assessments. Conduct re‑evaluations when indicated. Perform other duties as assigned. Requirements California State licensure as a Physical Therapist. Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advanced Resuscitation Training. At least 2 years’ experience as a Physical Therapist preferred. Trauma Life Support, or Advanced Resuscitation Training. Travel between sites and other locations occasionally required. Must have a valid California driver’s license, an automobile, and proof of minimum levels of car insurance as required under California law. An acceptable driving record is also required.
