Job Description

Job Summary:

Provide specialized home health nursing care to patients in the Franklin and Macon County areas, ensuring high-quality patient outcomes through expert clinical skills and compassionate support. This role involves working with a dedicated team to deliver in-home services aligned with agency policies and patient needs.

Responsibilities:

• Assess patients' health status and develop personalized care plans adhering to home health standards.
• Perform skilled nursing procedures, including medication administration and wound care, within scope of practice.
• Document care activities accurately using the HCHB system and ensure compliance with regulatory requirements.
• Collaborate with interdisciplinary teams to coordinate patient care and facilitate smooth communication.
• Educate patients and family members on health management, medication, and safety procedures.
• Maintain professionalism and adhere to uniform policies, ensuring a reliable and professional appearance.

Qualifications:

• Current and unrestricted RN licensure in North Carolina.
• Minimum of 2 years of experience in home health and hospice nursing.
• Demonstrated ability to perform complex nursing procedures and assessments.
• Reliable transportation and a dependable vehicle for patient visits.
• Strong communication skills and the ability to work independently.
• Knowledge of the HCHB documentation system and home health regulations.
