Psychiatric RN Location: 10- 622 Eagle Rock Ave, West Orange, NJ. Salary: $85,600.00 – $96,300.00 annually. Work Hours: Full time, 40 hours per week. Sign‑on bonus: $5,000. 4‑day work week; one weekend day required. Position Overview As part of the core services team, the PACT RN provides nursing assessment, direct‑support services and primary health care to seriously and persistently mentally ill individuals enrolled in the PACT Program. Provides support and education to other team members in nursing/medical treatment and health care. Essential Job Functions Monitor and assess the mental and physical health status of people served. Develop nursing, comprehensive and individualized recovery plans with the team. Contribute to population health component: prioritize medical needs, track outcomes, facilitate follow‑up, recommend screening. Participate in clinical and non‑clinical team meetings, individual and group supervision, and pursue supervisory support regularly. Provides, Teaches and/or Monitors Teach skills supporting individual recovery and community integration. Coordinate services with community mental health, non‑mental health providers, and other medical professionals. Provide mental and physical health education, community living skill training, and counseling in natural settings. Assess service recipients’ concerns, symptoms, and medication response; monitor side‑effects. Educate patients and family on medication and illness. Provide 24‑hour on‑call coverage on a rotating schedule. Refer to medical doctors for physical examinations (first 3 months). Respond rapidly to crisis; accompany patients to local screening centers or emergency services. Manage symptoms and help clients identify patterns and coping methods. Recognize high‑risk medical and psychiatric conditions and act appropriately. Offer technical assistance and education to team members in psychiatric nursing and physical health. Screen for metabolic syndrome (HgbA1c, fasting glucose, lipid profile, blood pressure). Administer prescribed medications and injections; draw blood and prepare lab specimens when certified. Record medication administrations accurately in the Medication Record. Adhere to nursing and agency protocols for medication administration and storage. Consult with other service providers regarding assessment, treatment and rehabilitation needs. Assess daily living skills and teach budgeting, cooking, laundry, cleaning, shopping, hygiene, transportation, self‑advocacy. Apply Illness Management and Recovery (IMR) and other evidence‑based practices (motivational interviewing, CBT, teaching strategies). Support patients in finding and maintaining employment and/or education/training. Ensure timely, quality documentation in compliance with DMHAS/Medicaid standards. Assist family and social network in managing symptoms, reducing stress, and achieving wellness. Required Qualifications Valid NJ Registered Nurse license. Experience One‑year experience working with individuals with serious and persistent mental illness. Driver’s License, Vehicle Valid driver’s license required.
