RN Case Manager

Southern Hills Hospital and Medical Center seeks an experienced RN Case Manager to support coordinated, patient-centered care in our dynamic ER-focused environment. In this role, you will perform clinical and psychosocial assessments, lead discharge planning, and manage utilization to ensure quality, cost-effective outcomes. Working closely with physicians, nurses, and interdisciplinary teams, you’ll facilitate safe transitions of care, educate patients and families, and align services with regulatory and payer requirements.
