Job Title Hospice Certified Nurse Assistant, PRN. Job Overview Residential Home Health and Hospice is seeking a PRN Hospice Certified Nurse Assistant to provide patient care in Traverse City, MI. This role supports patients in Traverse City and surrounding areas, with a maximum schedule of 10 hours per week. Responsibilities Collaborate with an interdisciplinary team to manage and deliver patient care plans. Provide direct patient care, including assistance with daily living activities and comfort measures. Utilize tablets, mobile phones, and EMR software to document care and communicate with the team. Self‑manage time and schedule to meet patient needs. Adhere to infection control and safety protocols. Qualifications Certified Nursing Assistant (CNA) certification in the state of employment. High school diploma or equivalent preferred. Hospice experience preferred. Valid driver’s license and ability to travel approximately 20% of the day to patient locations.
