Program Overview Hope For Youth is a nonprofit organization for over 50 years specializing in child welfare services. The Nassau Prevention Program, known as H.E.A.R.T (Helping Enhance Attachments And Remain Together), provides family‑oriented, supportive and rehabilitative child welfare services to children and their families. The objectives of the program are: Averting disruption that could result in imminent placement of a child in foster care. Enabling a child placed in foster care to return to his or her family earlier than otherwise possible. Reducing the likelihood that a child discharged from foster care would return to such care. Responsibilities Function within policies and procedures as outlined in the program practice guide and employee manual. Overall administration of the program including planning, organizing, launching, and ensuring its continued success. Collaborate with clinical supervisors for delivery of clinical services. Develop and implement policies and procedures to ensure ongoing success in accordance with HFY practices. Meet weekly with the Program Supervisor. Communicate with Nassau DSS and the court as needed. Gather program statistics and prepare reports for submission. Participate in treatment team, case review, and other meetings. Collaborate with referral resources and other agencies working with the family and the court. Work as part of an interdisciplinary team. Provide support for other projects and locations; travel as needed. Sit in on agency committees such as Training and Staff Development and Continuous Quality Improvement. Adhere to state regulations and applicable code of conduct. Perform other work necessary to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations. Qualifications LMSW with at least 7 years supervisory experience. Knowledge of and experience working in child serving systems. Excellent communication and leadership skills. Strong written, verbal and computer skills. Flexible work schedule to accommodate program needs. Valid NYS Driver's License in good standing. Physical requirements: regular access to reliable vehicle for work travel, ability to attend home visits, work from various HFY locations and conferences, read/write reports and log entries, walk/stand, bend/stoop, kneel/squat, climb stairs, reach overhead, repetitive wrist/finger movements, routinely lift 10 lbs and occasionally lift up to 50 lbs.
