Physical Therapist (PT) - Home Health Case Manager Location: Chicago - South Side (servicing ZIP codes 60620, 60628, 60643 and surrounding areas) Schedule: Day shift with rotating weekends and holidays Sign-On Bonus: Eligible for up to $10,000 sign-on bonus for external candidates. • Provide comprehensive physical therapy assessments and develop individualized care plans
• Deliver in-home therapy focused on improving mobility, safety, and independence
• Serve as a case manager, coordinating care with physicians and interdisciplinary teams
• Educate patients and families to support recovery and long-term outcomes
• Travel throughout the assigned North Side Chicago territory

What You'll Need

• Licensed Physical Therapist in Illinois (or eligible)
• Minimum 1 year of clinical experience (or strong clinical affiliation experience)
• Experience with home-based therapy equipment and assistive devices
• Strong communication, documentation, and time management skills
• Ability to work independently while collaborating with a broader care team
