Job Description
Job Title: LPN Home Health

Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience (depending on specialty)
License: Must have a current license in hand
Certifications: CPR, BLS
Must-Have: Proficient in HomeCare HomeBase EMR and Oasis


Description:
Provides coordination and direction for the provision of home nursing care according to physician orders.
Utilizes agency policies and procedures through the competent application of the nursing process.
Reports directly to the Clinical Manager.
Supervises and evaluates Home Health Aides and Licensed Practical Nurses.
Conducts home visits and attends periodic office meetings.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Points per week are broken down into various activities, with a total of 30 points expected.
Each type of visit and activity has a different point value.
Contractor will be assigned between 5-10 patients per day, equating to roughly 6-8 points.
No guaranteed hours; if the patient census is low, discussions for floating to another clinic are encouraged.
Types of cases include Start of Care, Wound Care, and Follow-up Visits.
Must be open to traveling up to a 75-mile radius from the assigned care center.
Mileage is reimbursed at the IRS standard rate.
Commute from home to the first and last patient is not billable.
Documentation must be completed within 24 hours of each patient appointment.
Delayed or incomplete documentation affecting billing and patient care will result in cancellation.
Accepted candidates must confirm readiness for driving to patient homes for daily visits and clinic meetings.
Candidates should be comfortable with drives that may exceed 20 minutes between patients.
Weekly productivity expectations require clinicians to hit 30 points, which corresponds to 40 hours of work.
All overtime must be pre-approved.
Candidates must be local or willing to travel.
Declining patients may lead to cancellations.
Experience in home health is required; hospice experience may also be applicable based on specialty needs.
