Job Summary The Care Management Care Coordinator is a key member of the Care Management team, responsible for supporting the day‑to‑day functions of a complex medical network, including outreach, case assignments, communication, referrals, and tracking of member needs. The role involves planning, prioritizing, and coordinating care services while serving as the primary contact for internal and external stakeholders. Key Responsibilities Support the general case‑management process: initiate outreach cycles, document interactions in electronic records, and identify issues that may impact timeliness of notifications to providers. Track, manage, and triage member needs within defined intervention periods, completing follow‑up responsibilities within required timeframes. Educate and coach members, families, and caregivers about care plans, health status, transitions, and insurance benefits to promote self‑management. Coordinate health‑care services: assist members with scheduling appointments, tests, screenings, and transportation. Assist with special projects and outreach campaigns, collaborating with outside stakeholders and vendors to coordinate member care. Enter data accurately into MedHok, Casetrakker, Jiva, and other databases; complete general office duties and fax tasks as required. Support clinical initiatives and quality‑measure programs, participate in outbound calls to members with potential quality triggers, and provide process support for assigned service areas. Qualifications Education : High school diploma or equivalent (minimum). Associate’s degree in business or a related administrative/clinical field is preferred. Experience : 3–5 years in a professional setting; experience in a health‑care or managed‑care environment is highly desirable. Internal applicants must have experience with Casetrakker, JIVA, or MedHok; external applicants should demonstrate a capacity to become proficient within three months of training. Skills : Strong knowledge of clinical services and organizational operations; proficient in Microsoft Word, Excel, email, and intranet; familiarity with PowerPoint, Access, Visio, and related software is desirable. Excellent written and verbal communication, organizational, and problem‑solving skills are required. Ability to handle multiple priorities, exercise sound judgment, and work independently as well as collaboratively across organizational levels. Working Conditions The role is performed in a normal office environment with the option to work from home as needed. It requires simultaneous use of a telephone/headset and PC, extended periods of sitting, and occasional lifting of 5–25 pounds. Additional hours beyond the standard schedule may be required, and a hoteling/remote work setting may present environmental distractions. Compensation & Benefits The annual base salary range for this position is $23.47 – $35.21 per hour. Compensation is based on role scope, skills, education, and experience, and may include variable pay.
