Hospice Registered Nurse Case Manager

Heart n Home Hospice, part of LHC Group within the Optum family, is hiring an RN Case Manager to oversee the plan of care and ensure quality hospice services in the community. You will perform initial evaluations, coordinate care, document progress, and collaborate with the IDG. Requirements include RN licensure in the state, a valid driver's license or reliable transportation, CPR preferred or obtainable within 90 days, and 1+ years of clinical experience. #J-18808-Ljbffr
