Medical Chart Prep Clerk 1 Year Experience

Job Description

The Medical Chart Prep Clerk is responsible for ensuring patient charts are complete and ready for upcoming office visits. This includes reviewing the patient's last office note, verifying that all ordered tests, procedures, referrals, and consultations have been completed, and confirming that all resulting reports have been received and properly scanned into the patient's medical record.

When reports or documentation are missing, the Medical Chart Prep Clerk contacts hospitals, imaging centers, laboratories, specialists, and other healthcare facilities to determine whether the recommended procedures were performed and to obtain the necessary records. All findings, follow-up actions, and communications must be accurately documented in the patient's chart to ensure continuity of care and support provider decision-making during patient visits.
