Minimum Requirements

Associate‘s, Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
Active Washington State Physical Therapist license required to start the assignment.
Minimum 1 year of home health physical therapy experience required.
BLS certification from AHA or ARC may be required.
Comfort with patient-to-patient travel during the workday; mileage reimbursed between patient visits (client policy).
Willingness to follow client dress code: navy blue top and scrubs with khaki shorts, per site policy.
Preferred: additional home health experience beyond the minimum.

Facility Type: Home Health.
Shift Name: Days.
Schedule: 5x8 shifts, days (start and end times provided by facility).
Weekly Hour Guarantee: 32 hours.
Productivity Expectation: 30 points.
Coverage Areas: Centralia and surrounding areas; candidate should be able to travel between patient homes within the coverage area.
On-call/Weekend Requirement: No weekend or on-call requirement.
