Work Environment

This role is primarily based in the homes of patients, providing therapeutic services in a comfortable and familiar setting. Key Responsibilities

• Evaluate, devise, and implement customized treatment programs for patients as referred by their physicians.
• Conduct comprehensive assessments and develop measurable goals to track patient progress.
• Utilize advanced therapeutic techniques and evidence-based practices to promote recovery.
• Deliver hands-on care directly to patients in their homes, ensuring comfort and compliance with treatment protocols.
• Provide consulting, education, and ongoing support to patients and their caregivers, empowering them through their recovery process.
• Collaborate closely with a multidisciplinary team, sharing insights and strategies that enhance patient care and outcomes.
• Meet utilization goals and participate in quality improvement initiatives that align with our organizational objectives.

Patient Population / Client Focus

You will have the opportunity to work with diverse patient populations, covering a wide array of specialties, including orthopedic, pediatric, neurologic, and sports medicine cases. Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Must hold a valid state license as a Doctor of Physical Therapy (DPT), as awarded by the Physical Therapy Examining Committee.
• A minimum of two (2) years of relevant experience in a clinical setting.
• Current CPR certification to ensure the safety and well-being of patients.
• Exceptional written and verbal communication skills to facilitate effective interactions with patients, families, and healthcare staff.
• Proficient in using word processing and computer systems to document patient care accurately.
• A strong ability to build rapport and establish trusting relationships with patients, enabling them to exceed their functional goals.
• A demonstrated passion for improving the lives of patients through meaningful connections and dedicated service.

Preferred Experience

While we require a minimum of two years of experience, candidates with advanced competencies in specialized areas such as manual therapy, pediatric care, or neurologic rehabilitation will be considered for additional responsibilities and growth opportunities within the practice.
